Medical Verification (Malpractice Insurance)
Be advised that the Risk Management Division does not track individual physicians or their work locations. However, the process of obtaining verification of malpractice & claims history is straightforward. Once you’ve identified the RMD-insured agency that the medical professional was associated with, you can easily contact the relevant agency to get the information for which you’re looking. The agencies to contact are shown below:
University of NM Hospital (UNMH): You may send your requests directly to risksvcs@unm.edu. Unfortunately, their office does not provide malpractice coverage or cliam history verification. Visit their website to find out whom you should contact: https://risk.unm.edu/medical-credentialing.html.
Miners’ Colfax Medical Center (MCMC): All requests may be submitted to Lonnie Medina at (575) 445-7790 or by email at lmedina@minersshop.com.
NM Department of Health (NMDOH): All requests may be submitted to Adreien Jaramillo at (505) 467-9447 or by email at adreien.jaramillo@doh.nm.gov.
You may also contact the State of New Mexico Medical Board (NMMB) at (505) 476-7220 or toll free at (800) 945-5845. You may also fax your request to (505) 476-7237 or email it to nm.medicalboard@nmmb.nm.gov.
Special Events Insurance
We do not accept verbal requests for Certificates of Insurance. To request coverage for an event, provide a copy of the venue request form (or similar document) and submit the following information below via email to GSD-RMDunderwriting@gsd.nm.gov:
- Name of State Agency, Department and Bureau
- Name of the event
- Start date/time and end date/time of coverage needed
- Address or location of the event
- Certificate holder/loss payee information, including address
- Type of coverage needed
Evidence of Coverage
There are three ways to receive a copy of an agency’s Evidence of Coverage:
- Contact the agency directly
- Obtain access to the RMD Underwriting Portal
- Email us directly at GSD-RMDunderwriting@gsd.nm.gov
Mexico State Vehicle Travel Certificate of Insurance
If you are using a NM State vehicle and traveling to Mexico, you will need a separate certificate of insurance for each vehicle. Please use the following steps:
- Contact James Chavez to obtain a waiver for travel to Mexico (505) 660-5562
- Send a waiver and the following information to Risk Management Division at GSD-RMDunderwriting@gsd.nm.gov:
- Vehicle make, model, year, and VIN number
- Destination location, dates of travel, name of the department or bureau
- Traveling contact person name
- Phone and fax numbers coordinating the travel
Additional or Third Party Insured
Pursuant to Article IX, Section 14 of the New Mexico Constitution (anti-donation), RMD will not indemnify any private or public sector contractor, entity, business, person, or organization as an additional insured, nor will RMD indemnify, hold harmless, or defend any 3rd party individual or business.
